Connect Vulnerable Patients to Local Support

LinkZorg is a digital platform designed to help emergency staff at OLVG Oost Hospital in Amsterdam connect 3U patients—those who are unhoused, undocumented, or uninsured—with essential follow-up care.

Systemic Gap

In Dutch ERs, “3U” patients—uninsured, undocumented, unhoused—often fall through the cracks after receiving acute care. Without stable housing, phones, or internet, existing digital solutions fail them.

Design Response

Together with hospital staff and NGOs, we built LinkZorg: a paper-first, digitally supported platform. Doctors filter services in seconds and hand patients a printed care plan—no data storage, no stigma, only relevant support.

Our Mission

We believe that every patient deserves comprehensive care, and that care providers should have easy access to all available resources. By creating a trusted platform for resource connection and care planning, we aim to improve the well-being of Amsterdam's unhoused, undocumented, and uninsured residents. We aim to empower patients and hospital workers, and to support Amsterdam's care network.

We are Mihaela Chiselită, Daniel Klein, and Amanda Wee, designers from AUAS Master of Digital Design. Our shared focus is on making complex systems more humane through dignity, accessibility, and care. With LinkZorg, we designed a practical tool that bridges healthcare gaps for vulnerable patients.
